Annotation: Featuring two communities participating in the Using The Community Guide for Community Health Improvement pilot initiative, this archived webinar offers an overview of the initiative to date and highlights the stories of those participating. Led by WellSpan Health (York, PA) and INTEGRIS Health (Oklahoma City, OK), coalitions involving the local health departments and numerous other stakeholders in both communities have been using the population health driver diagram framework to take action and implement community health improvement activities to address behavioral health needs. During this webinar, representatives from WellSpan Health and INTEGRIS Health shared their experiences with the initiative over the past year, including successes and lessons learned. In addition, background information about The Guide to Community Preventive Services (The Community Guide) and the population health driver diagram framework was provided.

Annotation: This recorded webinar session discusses the results of the summer 2016, AMCHP-led assessment of the Title V workforce that assessed the scope, staff, and training needs of state Title V programs. The recorded webinar was developed to provide background to a interactive session to discuss the results shared during the recording, identify gaps in the assessment, share best practices and resources, and provide recommendations for future assessments.

Annotation: The MCH Navigator has developed a new, online self‐assessment tool for the Title V workforce. This session described the process utilized to convert the assessment from its original design into an electronic format including an innovative conditional programming structure, customized results page, and other new features. It discussed how Title V professionals can use the tool to identify their strengths and areas of need in serving the MCH community. The tool was structured using the 12 MCH Leadership Competencies with each question addressing participant knowledge or skills. Therefore, results and learning opportunities suggested based on specific needs are directly correlated to the MCH Leadership Competency framework developed by MCHB.

Annotation: This skills‐building session provides participants with the knowledge and skills to determine the most appropriate NPMs based on state needs as part of the transformation of Title V; the workshop also focuses on development of state‐initiated Structure/Process measures. The transformation of Title V is almost complete and signifies a new era for MCH programs. During 2014‐15, state Title V programs will be completing five‐year needs assessments to select a new set of state MCH priorities and state performance measures; with the implementation of MCH 3.0, states also will select a subset of national performance measures (NPMs). In 2016, each Title V program will need to develop state‐initiated Structure and Process Measures to directly measure their impact on the NPMs.

Annotation: This skills‐building session provides state Title V directors and staff with practical information on how to operationalize the transformation of the Title V MCH Services Block Grant, with a focus on the changes impacting the submission of the fiscal year (FY) 2016 application, including the five‐year needs assessment, and FY 2014 annual report. The session focuses on the following areas: 1) new information system for the Title V Information System (TVIS); 2) new guidance for the MCH Block Grant Application Narrative; 3) new guidance for the MCH Block Grant National Performance Measures; and 4) new guidance for other parts of the MCH Block Grant application and annual report.

Your CHNA will provide valuable data to direct resources toward high-risk populations, identify opportunities to enhance care delivery, inform patient engagement initiatives and direct overall strategic planning. In this joint Hospitals in Pursuit of Excellence (HPOE) and Association for Community Health Improvement (ACHI) webinar, CHNA experts discuss ways to build upon your previous CHNA to bring more value to your community and your hospital. Specific examples are presented on how hospitals are using CHNAs to inform population health management initiatives and on the intersection between community health needs and care delivery.
