In the patient and family centered care model of health care, patients and their families are members of their care teams, not just bystanders. University of Michigan Health System staff and patients talk about what patient and family centered care means to them. Research shows that including patients and their families in the care process results in better outcomes, including reduced length of stay and readmissions.
