Module 8.3: 5 Implementation Strategies

In this module we will augment the knowledge you've gained from the last module's learning opportunities by providing you with 5 implementation strategies gathered from our team of experts.

These implementation strategies follow a conceptual model of widening circles of influence. In this model, MCH leaders utilize resources and tools to activate change within their organization, which in turn incorporates partners through its systems of influence. Together, changes may be implemented to affect specific target populations and the MCH community in general.

These 5 implementation strategies align with the 5 circles of the Model for Public Health Competency Implementation, and represent ways that you can utilize what you've learned over the past few modules. In particular, we have included resources and strategies to align your work with the transformation of the MCH Block Grant.

  1. How to Advance Yourself as an MCH Leader (Self-Reflection Strategy). Identify your personal strengths and areas of growth by taking 5 minutes to register/log-in to the MCH Navigator's Self-Assessment and answer questions related to Competency 8: Family-Centered Care.
  2. How to Find and Use Tools to Help You (Information Strategy).Although the concept of family-centered care (FCC) is now widely considered the standard of care for all children, regardless of health status, the benefits of FCC are especially important for families of children and youth with special health care needs (CYSHCN). These standards represent the consensus of a consortium of public and private organizations participating in the first-ever national, collective effort to detail the components of a high quality system of care for CYSHCN. They can be used or adapted by a range of national, state, and local level stakeholder groups including state Title V CYSHCN programs, health plans, state Medicaid and CHIP agencies, pediatric provider organizations, children's hospitals, insurers, health services researchers, families/consumers, and others.

    Find other resources that health professionals, families, schools, and others can use to develop family-centered systems of care for CYSHCN in the National Center for Education in Maternal and Child Health's knowledge path.

  3. How to Activate Your Organization (Organizational Strategy). Evolving organizational culture to align with the principles of family-centered care (FCC) takes time and resources. Along the way, it is important to understand and demonstrate the ways partnering with families adds value. Would you like to assist your organization in measuring the impact of FCC implementation? Begin with this 4-step guide from the National Partnership for Women and Families. Topics include setting goals, identifying what to track, monitoring progress, and determining how to document and share results.

  4. How to Incorporate Partners (Systems Strategy). The health care system in the United States is in the process of rapid transformation, and the idea that everyone should have a patient- and family-centered medical home (PFCMH) is gaining broad acceptance. This integrated information system is designed to help state and family leaders access data on how children and adolescents experience care within medical homes. Use the system to find your state's medical home performance profile for all children and adolescents or for those with special health care needs, compare data across all states, or view state ranking maps. Learn about national projects, initiatives, and policies focused on advancing the pediatric medical home model. Discover tools, and resources to help practices provide family-centered care. Then, have a conversation with your partners about opportunities for improvement.

    Would you like to advance your knowledge and understanding of family-centered care within the pediatric medical home? This resource defines family-centered care (FCC), describes characteristics of FCC, and provides tools to help clinicians and practices provide FCC.
  5. How to Engage Your Communities. This model of local (county) coalitions, along with a statewide learning collaborative, is helping to develop interagency solutions to improve communication and interaction across agencies serving children in California. Check out the slideshow and tools developed by member coalitions and others, as well as key literature on family-centered care coordination.
